*Position will cover Atlantic Health Newton Medical Center & Atlantic Health Hackettstown Medical Center.*
Performs basic, routine duties related directly and indirectly to clinical care of military veteran patients and their family members.
1.Runs daily admission report to check if new patients who are military veterans have been admitted into the hospital.
2. Reviews charts, communicates with clinical team, patients and family members to determine need for resources. Assist with linkages to resources in the community.
3. Meets and greets identified military veteran patient admitted into the medical center at bedside if appropriate.
4. Assist clinicians with care coordination tasks such as calling, emailing and/or faxing information to PCP or other providers, with a signed ROI from patient.
5. Discusses patient's treatment plan, symptoms, and other pertinent information with patient's care team including outside providers, with a signed ROI from patients.
6. Assists clinicians with sending (email or fax) and managing disability and human resources forms and/or information for patients who are in treatment.
7. Appropriately documents patient contacts, phone calls, group notes, and other patient information in EMR as necessary.
8. Assists with inputting evidenced-based screenings and other intake documents in EMR as assigned.
9. Participates in multi-disciplinary treatment team and general business meetings.
10. Orients patient to program and develops rapport with patients, family and significant others as needed.
11. Applies principles of infection control and safety in the work environment.
12. Assists in keeping patient care and non-patient care areas clean (e.g., wipes tables, neatens storage rooms, pantries and other areas).
13. Safely and effectively utilizes the hospital computer system in accordance with hospital policy.
14. Attends community and stakeholder meetings related to military veterans as a representative for True North
15. Participates in orientation of mental health assistants.
16. Completes other responsibilities as assigned
1. Bachelor's degree required
3. Previous experience in a behavioral health setting preferred
4. Previous experience working with Military veterans preferred.
5. Previous direct patient care experience preferred.

2. Excellent communications skills
3. Good working knowledge of Microsoft Office Suite including Word and Excel, and Microsoft Teams.
4. Ability to read and interpret documents.
